Online GLP-1 Programs in North Carolina
North Carolina stands out: it is one of the few states whose Medicaid actually covers GLP-1s for weight loss. The path was bumpy, with a brief 2025 cut and a December reinstatement, and it sits alongside a striking paradox, the state employee plan dropped the same drugs the year Medicaid added them.

Does North Carolina Medicaid cover GLP-1s?
North Carolina Medicaid is one of the few state programs that covers GLP-1s for weight management, a benefit it added effective August 1, 2024 for beneficiaries 12 and older. Coverage was briefly eliminated on October 1, 2025 during a budget stalemate, then reinstated effective December 12, 2025 (NC Medicaid, 2025). Wegovy is preferred; Zepbound and Saxenda are non-preferred and require step therapy, and prior authorization applies. GLP-1s for type 2 diabetes are covered separately.
Online Access
Getting a GLP-1 online in North Carolina
GLP-1s are not controlled substances, so North Carolina allows them to be prescribed via telehealth. The medical board permits establishing a provider relationship through synchronous or asynchronous telehealth without a prior in-person visit when the standard of care is met, though prescribing based solely on a static online questionnaire is not acceptable (NC Medical Board). The prescriber must hold a North Carolina license.
Compounding
Compounded semaglutide & tirzepatide in North Carolina
Routine compounding of semaglutide and tirzepatide ended after the FDA resolved the shortages in early 2025 (FDA, 2025). North Carolina adds no special carve-out beyond the federal 503A/503B framework, so compounding is limited to documented, patient-specific clinical need.
Demand
Where GLP-1 demand is highest in North Carolina
About 35% of North Carolina adults have obesity and roughly 13% have diagnosed diabetes (CDC BRFSS via America's Health Rankings, 2024). Demand centers on Charlotte and the Research Triangle of Raleigh and Durham. North Carolina expanded Medicaid in December 2023, enrolling nearly 600,000 people in the first year, and unlike most states it covers weight-loss GLP-1s, an unusual level of access.
Questions
North Carolina GLP-1 FAQ
Does North Carolina Medicaid cover Wegovy?
Yes. NC Medicaid covers GLP-1s for weight management (Wegovy preferred; Zepbound and Saxenda with step therapy and prior authorization). Coverage was briefly cut in late 2025 and reinstated effective December 12, 2025 (NC Medicaid, 2025).
Can I get a GLP-1 prescribed online in North Carolina?
Yes, by a North Carolina-licensed clinician. The medical board allows a telehealth relationship without a prior in-person visit when the standard of care is met, but a static questionnaire alone is not enough.
Why did state employees lose coverage while Medicaid added it?
They are different programs. The NC State Health Plan for employees dropped weight-loss GLP-1 coverage in 2024 over cost, while NC Medicaid added it for beneficiaries that same year (KFF Health News, 2024).
