SS-31 (Elamipretide) Dosage Chart
SS-31 (elamipretide) is a mitochondria-targeted tetrapeptide that binds cardiolipin on the inner mitochondrial membrane and is studied for restoring mitochondrial bioenergetics. It has been investigated in registered trials for primary mitochondrial myopathy, Barth syndrome, heart failure, and dry AMD, none of which has led to approval. One key point up front: the grey-market research dose (1-5mg per day) sits far below the 40mg per day used in every human trial.

Dosage Protocols
Each row reflects a common research protocol variant. These are reference ranges derived from published literature and community research — not medical guidance.
Protocol 1
Grey-Market Research Convention
Dose
1-5mg subcutaneous once daily
Duration
4-12 week blocks
The dose most research-peptide protocols circulate. It runs 8 to 40 times below every dose used in registered human trials, so read-across to clinical outcomes is unsupported. Listed here for transparency, not endorsement.
Protocol 2
Clinical Reference Dose (MMPOWER-2 / MMPOWER-3)
Dose
40mg subcutaneous once daily
Duration
4 to 24 weeks
Karaa et al. dosed 40mg per day SC in primary mitochondrial myopathy (MMPOWER-2, 2020; MMPOWER-3, 2023). Both largely missed their primary walk-distance and fatigue endpoints, but they establish 40mg per day as the studied dose, with injection-site reactions as the main tolerability issue.
Protocol 3
Other Indication Reference (Barth / dry AMD)
Dose
40mg subcutaneous once daily
Duration
12 weeks to 168+ weeks (open-label extension)
TAZPOWER (Barth syndrome, Thompson et al. 2024) and the dry-AMD program used the same 40mg per day SC dose, confirming it as the consistent dose across every elamipretide trial.

Handling Notes
Lyophilized: -20°C long-term, 2-8°C short-term. Reconstituted with bacteriostatic water: refrigerate at 2-8°C and use within 14-30 days. Protect from light, swirl gently, never shake or freeze the reconstituted solution.
Research Considerations
Cautions
- The clinical-vs-research dose gap is the headline caution: every human trial used 40mg per day SC, while grey-market 1-5mg per day protocols have no efficacy or safety data behind them.
- Not FDA approved for any indication. MMPOWER-3, the pivotal myopathy trial, missed its primary endpoints, and the AMD and heart-failure programs also failed key endpoints.
- Injection-site reactions and erythema were the most common adverse events in trials. The compound was otherwise generally well tolerated at 40mg per day.
- Identity and purity of grey-market SS-31 are unverifiable without a batch-specific COA. Acetate salt content and actual peptide mass vary between suppliers.
